Graduate Medical Education Applying for a Standard Fellowship

Applicants to the standard three-year fellowship should first download and review these application instructions.

As part of the application, each letter of recommendation must be submitted with a completed Pediatric Cardiology Evaluation Form. Please ask your letter writers to complete the appropriate form below and include it with their letter:

• Pediatric Cardiologist Letter Writer: Download the evaluation form.
• Residency Program Director or Associate Program Director Letter Writer: Download the evaluation form.

Please note: A completed Pediatric Cardiology Evaluation Form is required for every letter of recommendation.

All standard fellowship application materials must be submitted through the Electronic Residency Application Service (ERAS) website.

Timeline for application and selection, per NRMP:

1. Deadline to upload a completed application to ERAS website (for fellowships beginning July 1 of the following year): The deadline date, in July, varies each year; please refer to ERAS website for the date and other information.
2. Interviews: Aug. 1 through Oct. 31.
3. Final selection: A schedule of dates and other information can be found by visiting the NRMP website.
